Katie Hoffman, RN

Director of Nursing

Katie Hoffman, RN Director of Nursing, has spent her whole career in healthcare, starting with obtaining her CNA while in high school. She went on to get her EMT Basic while going through nursing school, before meeting a Wyoming cowboy & relocating to Sublette County 15 years ago. Katie has been a long-time face of Sublette Center from a floor nurse, to transitioning to MDS Coordinator for most of her 10 years at Sublette Center, until recently officially taking on the role as Director of Nursing in November of 2023. Katie filled in for the role of DON a great deal over the last decade & is excited to continue be a part of the care of our seniors who hold such a dear place in her heart. Katie received the DAISY award for Excellence in Nursing in 2019. Her greatest love of Sublette Center is the home like, family centered environment that brings our community together honoring those who created the communities we are all lucky enough to raise our families in today. Katie & her husband Dusty have 3 kids; Lexy, Stetson & Paisley who are their pride & joy.  Her family’s roots are in agriculture with kids who participate in 4-H, Jr Rodeo,  Sublette County Rodeo Royalty, all things ranching & many sports. If Katie is not at Sublette Center, you will find her with her family supporting her kids & husband in all of their endeavors.
